How to upload payment Details
1. Proof of Payment
Screenshot or Save the image of your bank transfer, GCash receipt, deposit slip. Please see the image below for reference.
Fill-up the form on the right side with your name and your email address. Make sure you choose the correct type of payment (Homecoming, Membership, T-Shirt, Polo Shirt or Donation) and Mode of payment (Metrobank or Through Secretariat)

Click the Choose File Button and attach your payment details image you used and click the open button in the dialogue box.
Click Submit Button and wait for the form response “Thank you [your-name] for your payment. It has been recorded”.
